Pre Admission
PREPARING FOR YOUR ADMISSION
We suggest that you use the following guidelines as a handy checklist of items that will help to prepare you and assist you on the day of your admission.
- To help us prepare for your arrival, please post or drop in your completed pre-admission form to us as soon as possible, a minimum of one week prior to your admission. If you don't have this form, contact the hospital by telephoning (02) 60411411 and we'll send you one.
- You will need to contact the hospital 24hrs prior to your admission to confirm your admission and fasting times as well as making sure you are ready for the day. Should you have any questions or queries related to your admission, please contact us on 1800 224242
- If there is any change in your physical condition, such as a cold or fever, please let your doctor know immediately as your procedure may need to be rescheduled until you are well again.
- Please do not wear make-up, nail polish or contact lenses.
- Please leave all jewellery and valuables at home as the hospital will not accept responsibility for their loss. A small amount of cash for incidental items is recommended
- Please do not bring your mobile phone to the hospital as the signal may interfere with electronic medical equipment. All rooms have a direct dial phone for you to receive and make calls.
- Should you need to bring any electrical equipment into the hospital it will need to be checked by our maintenance staff prior to it's use.
WHAT TO BRING
- Any letter(s) from your doctor
- Relevant X-rays, scans or pathology reports
- Current medications in their original packaging including tablets, creams, inhalers, mixtures, vitamins or drops this includes any herbal or natural remedies.
- Private health insurance details
- Medicare card
- Veteran's Affairs details
- Pharmaceutical entitlement number
- Full/part pension number
- Workcare or TAC information including your employer's details and claim numbers
- Slippers/scuffs, sleepwear and dressing gown
- Toiletries, soap, toothpaste and toothbrush
- Glasses, hearing aids and non-motorised walking aids
- Name and contact details of your General Practitioner
- Name and contact details of your next of kin and a second contact person
- If you have an Advanced Health Directive and/or an Enduring Power of Attorney for health matters, please bring a certified copy of these with you.
Getting To The Hospital
Albury Wodonga Private Hospital is located in West Albury in Pemberton Street (corner of Padman Drive). Free off street parking is available in the hospital's Visitor's car park.
A regular bus service to West Albury operates for those requiring public transport. Timetables are available from reception.
